Social Adult Day Program Coordinator
This position is part time (21 hours per week) and will be based out of the Avis/South Shore JCC building. The responsibilities are as follows:
  • Coordinate and supervise day to day operations of the program
  • Ensure that a complete and accurate Care Plan is provided for each client in accordance with DFTA standards.
  • Coordinate transportation services for client to and/or from the program
  • Maintain caseload of 20, including maintenance of daily attendance logs, completion of quarterly care plans with each client and his/her caregiver, etc.
Requirements:  BSW or Bachelor’s in related field required, minimum of 1 year experience in social service field preferred.
